Patient Records
All patient consultation and medical records are kept strictly confidential. Only authorised personnel have access to personal data. A copy of your medical history may be issued to other doctors on the patient’s request. This can only be done with a signed release from you.
A release form can be obtained from all medical clinics, Clocktower Medical Centre will transfer all patient medical histories electronically under very strict controls.
Please note: A fee will be charged for the transfer of medical notes to cover administration time and postal costs.
